POSITION: Case Manager Denver County Kinship Program
DIVISION/PROGRAM: Kinship program
QUALIFICATIONS: Bachelor’s degree in a human service related field plus two years of experience working with families in their homes or a comparable mix of education/experience required. Must be committed to and skilled at building relationships with under-resourced, at-risk families. Must be comfortable going into homes that may be cluttered, dirty, dark, and not air-conditioned. Must be open to learning and adapt well to change. Must be able to be both creative and flexible in service delivery of Case Management and Case Manager duties. Must be open to supervision and feedback. Must have strong computer skills; prefer Word, Outlook, Access, CBMS and/or other web-based systems. Prefer knowledge of metro Denver/County area resources and or be willing to learn. Must be able to work independently in clients’ homes and in the community. Must be able to establish and maintain working relationships with various community providers and team members. Must have access to acceptable transportation on a daily basis for home visits and various off-site meetings. Must have a valid
POSITION SUMMARY: Case Manager for the Denver County Kinship Program
To increase the stability of families receiving Child-Only TANF by providing time-limited case management, including needs based assessment, crisis intervention, referrals to community resources, financial assistance, follow-up, and emotional support. To track data pertinent to the above activities, the client population, and the impact of the service.
