POSITION: Bilingual SafeCare Case Manager
DIVISION/PROGRAM: Child and Youth Services-SafeCare Program
QUALIFICATIONS: Bachelor’s degree in a human service related field, with 2 years of experience providing case management or home visiting services strongly preferred. Strong interpersonal, communication, and organizational skills required. Computer proficiency required, with skills in MS Word, Excel, Access, and web-based database preferred. Knowledge of community resources in Metro Denver preferred. Bilingual (English/Spanish) oral and written skills required. Must be able to work independently in clients’ homes and in the community, and develop and maintain working relationships with various community providers. Must have access to acceptable transportation on a daily basis for home visits and off-site meetings. Must have a valid Colorado driver’s license, car insurance, and acceptable motor vehicle record.
POSITION SUMMARY: Provide home-based case management to parents/caregivers of children aged 0 – 5 years, including assessment, crisis intervention, referrals/linkages to community resources, financial assistance, follow-up, and supportive services to enhance and support family’s participation in SafeCare Colorado services.
SCHEDULE: Full-Time-40-hours, Non-exempt, Monday-Friday, may include evenings and weekends.
