Bilingual Case Manager
POSITION: Bilingual Spanish-English Case Manager
DIVISION/PROGRAM: Community Family Resource Team (CFRT)
POSITION SUMMARY: To increase the stability of families in Adams County who are struggling with teens that are beyond parental control by providing time-limited home-based case management, including assessment of family needs, crisis intervention strategies, referrals to community resources, follow-up, and emotional support. The case manager will field incoming crisis calls and offer in-home services, which may include referrals to community resources and/or family crisis intervention. The case manager will develop and maintain a list of community resources, facilitate quality assurance of cases, and track data pertinent to the above activities, the client population, and the impact of the service.
QUALIFICATIONS:
- Bilingual in Spanish and English, capable of providing all services and communication necessary in both languages.
- Bachelor’s Degree in human services-related field, or a comparable combination of education and experience.
- Minimum 1 year experience providing case management, crisis intervention, or related services with at-risk youth and families.
- Knowledge of Adams County and other metro Denver area resources strongly preferred.
- Access to acceptable transportation on a daily basis, for various home visits and off-site meetings.
- Valid Colorado drivers’ license, car insurance, and acceptable motor vehicle record.
- Clear background checks with Colorado Bureau of Investigation (CBI) and the Central Registry.
